Instructions to Complete Student Information Forms

1. Log into the Parent Portal.
2. Select "Form(s) to Review" in the yellow bar at the top.

There will be two (2) forms per child to complete:
  • 2024-2025 Student Information Form
  • 2024-2025 Student Medical Information Form
3. For each form, select the green "Review" button to complete.
4. Once you have completed the form, select the blue "Submit" button.


We also ask that you update your child(ren)'s emergency contact information:

1. After logging into the Parent Portal, select each child's name (in the left corner), and then the "Contact Card" tab (this will need to be done for each child).
2. Scroll down to the "Emergency Contacts" section, and select "Manage Phones".
3. Select "Create a New Phone Number" and fill out the fields.
4. Select "Save"
